Q: I try to stay physically fit by going to my gym and eating a balanced diet. Can anything be done to improve my cellulite and maybe lose a couple of inches

Q: I’ve seen some stories in the media addressing the safety of out-patient surgery. Is out-patient surgery safe?

A: Out-patient cosmetic surgery, done in a certified facility by board-certified plastic surgeon, is very safe. Let's take tummy tucks for example. My experience, and I've done hundreds if not thousands of these types of operations since we opened our office surgery suite over 6 year ago, is that patients do BETTER at home. I routinely do multiple procedures, such as liposuction and breast augmentation (which takes about 2 hours) and send the patient home. Everyone is aware of the problems in hospitals with drug-resistant, bacteria, overworked nurses, etc. And I always insist on an adult to be at home with the patient attending to them. I also researched the plastic surgery data and found articles confirming my point above. Don't get me wrong, if you have general anesthesia for 4-6-8 hours, you probably should be in a facility overnight, but not for a routine 2-3 hour tummy tuck done with sedation and local. Quality patient care, safety and successful surgical outcomes are the result of the patient, the surgeon and the surgical staff working together. Be sure and select a real plastic surgeon, someone who is board certified in plastic surgery and a member of the American Society of Plastic Surgeons.

Q: Dr.Haas, I am 42 and need liposuction on my hips and thighs. I have been seeing a lot of ads for lipo dissolve that does not involve surgery. Can I expect the same result from shots as I can from liposuction?

A: The current fad is injections to melt away the fat, known as Lipo Dissolve or Meso Therapy. This requires multiple injections (hundreds) and many office visits. The results are minimal, compared to one day of surgery with dramatic results, most people will choose liposuction. We know liposuction is safe, effective and routinely done in our office with sedation. I think liposuction is still the better choice for getting the results you want.

Q: Saline vs. Silicone Breast Implants which implant is safer, what is the difference and how do I chose which implant is better for me?

A: Silicone implants were extensively studied by plastic surgeons and the manufacturers for 14 years and the FDA has now declared them safe for general use. If safety is one of your main concerns, I would refer to the full approval from the Food and Drug Administration, and be assured that all of our studies indicate silicone implants are safe. Other countries around the world did not remove silicone implants from their market and have virtually never used saline implants. Saline implants are filled with salt-water, and we are all about 60% saline. If a saline implant leaks, the bag will deflate and the saline would be absorbed. Silicone is a family of chemical compounds made from a naturally-occurring element. If a silicone implant were to leak, it would stay in the pocket and does not harm your body. Keep in mind when deciding between saline or silicone that there are risks and benefits to both. The most important factor in choosing your surgeon is to make sure they are Board Certified by the American Board of Plastic Surgery.

Q: I’d like improvement in my tummy and don't know if I need a tummy tuck or liposuction.

A: You may need either or both to get the best result. If you don't have lots of stretch marks or a lot of excess skin, sometimes liposuction alone will give you the look you want. If you've had big, long babies, your stomach muscles are probably stretched out along with your skin. That generally means you'll need a tummy tuck to get a flat belly. I almost always do a little bit of liposuction on the love handles and hips to accentuate the new waist. Liposuction is less invasive than a tummy tuck and recovery is shorter. Patients can expect to resume normal activity, with some soreness, in a few days to a week or so. Tummy tucks will result in a flatter abdomen, but recovery is longer. Expect 3 weeks off work and you will not be 100% for roughly two months. The scar is usually hip-to-hip with a tuck, but will today's techniques, is certainly worth the improvement in shape. The decisions on the above are very logical and will be explained fully in your consultation and pre-op visits. We want patients to be as educated as possible in regards to what to expect from certain procedures.

Q: I am 45 years old and have started noticing some loose skin on my jowls and neckline. I definitely am not ready to go under the knife. What can I do to minimize wrinkles and tighten my skin?

A: Technology has really improved over the past few years so we now have effective non-surgical options that have minimal downtime. Q: What is a mini-facelift?

A: A mini-facelift is a less extensive cosmetic surgery operation than a full facelift. The technique and the results of this procedure have improved tremendously in the past few years. The ideal candidate is usually younger than someone who needs the full facelift, which involves the neck more. Mini-facelifts are also what some people call lunch-time lifts, quick-lifts, etc.

Mini-facelifts are ideal for women who are just beginning to see their cheeks falling and some early jowls along their jawline. This can be in the late 30s to mid-50s and even beyond. Again, if you have a lot of loose skin in your neck, you probably would get a better result with a traditional facelift.

We do all of our facelifts in our JCAHO-accredited in-office cosmetic surgery suite in Louisville Kentucky. The procedure is done with the patient sedated so there is no awareness, no sensation, no memory, but does not involve general anesthesia and the risks associated with that. This operation lasts from 2-6 hours depending on the complexity and what other procedures we may do in combination, ie brow lift, eyelid lifts, liposuction, etc. Patients spend about an hour in our recovery room and then can go home with a responsible party. I personally call and check on every surgery patient the night of their procedure.

It is becoming more popular to combine some Botox and fillers such as Restylane or Juvederm during the procedure. Downtime can be as short as a weekend. Some bruising and swelling is to be expected but goes away on their own in a few days to a week. I am always impressed by how little soreness there is with the mini-facelift. I had a friend ask me why the mini-facelift is becoming more popular, and the easy answer is because it works.

Q: How to regain your shape after childbirth with a Mommy Makeover?. What is a mommy makeover?

A: A mommy makeover is a catchy term for the combination of procedures, usually including a tummy tuck / liposuction and breast lift / augmentation.The effects of pregnancy and childbirth and breast-feeding can ravage a woman's body...loose skin on her belly along with weakened abdominal muscles, loss of breast tissue with an 'empty sock' look, sagging breasts, etc. I'm seeing a lot of these patients and doing a lot of combination procedures. The combination of a breast lift and/or augmentation, along with a tummy tuck or lipo can bring back and maybe make an even better shape compared to pre-pregnancy. This is a great combination of changes the ratios of bust-line and waist-line and provides a flat belly. Of course this is surgery and all surgery has risks. Patients need to plan their schedule for their recovery. I tell people with the above procedures, or anything that includes a tummy tuck...expect to take 3 weeks off work, be about 75% better at one month, and not 100% for two months. We do all of our procedures routinely in our certified in-office surgery suite.
